A new study is now causing quite a stir.
A research team in Boston reports that certain multivitamin supplements could measurably slow biological ageing in people over 70. The study was published in the highly regarded journal Nature Medicine-and many international outlets are already pitching it as a kind of fountain of youth in tablet form. A closer look, however, paints a far more sober picture.
What the COSMOS study is actually about
The trial is called COSMOS, short for “Cocoa Supplement and Multivitamin Outcomes Study”. The central question was straightforward: does taking a daily multivitamin complex change specific biological markers linked to ageing?
About 1,000 people took part-roughly half women and half men-and all were at least 70 years old. Participants were randomly allocated to one of four groups and took capsules every day for two years:
- Multivitamin supplement plus cocoa supplement
- Cocoa supplement only plus placebo
- Multivitamin supplement only plus placebo
- Placebo only
No participant knew which group they were in. This double-blind design is considered the gold standard in medical research.
Epigenetic clocks: what did they measure?
The researchers were not looking at wrinkles or gym performance, but at so-called “epigenetic clocks”. This is a relatively new biological measurement approach.
Put simply, certain chemical tags on genetic material-most commonly methyl groups on DNA-tend to shift as people age. These patterns can be read from blood samples and combined into an estimate of “biological age”. If that differs markedly from a person’s actual age, it is interpreted as a sign of accelerated or slowed ageing.
The study used five different epigenetic clocks to test whether multivitamins could measurably influence biological age in the laboratory.
Participants provided blood samples at the start of the study, after 12 months, and after 24 months. The team then calculated-based on changes in these markers-whether ageing appeared to speed up, slow down, or remain unchanged.
The “four-month youth bonus” - what does that figure mean?
The headline result now circulating widely is this: after two years, those taking the multivitamin complex were, on average, about four months “younger” than the placebo group-according to the epigenetic clocks.
This pattern appeared across all five measured signatures; two of these are regarded as especially closely linked to mortality. The gap was largest among participants who already showed signs of accelerated ageing at baseline.
By contrast, the cocoa supplement produced no measurable benefit for these markers in this study. Any hope of cocoa as an anti-ageing miracle did not materialise here.
Four months: impressive, or rather underwhelming?
On paper, “four months younger in two years” sounds appealing. In day-to-day terms, the impact looks much less dramatic:
- These are laboratory measurements, not visible youthfulness.
- Statistical significance only means the effect can be detected; it does not mean it matters in everyday life.
- To extend healthy life by a noticeable amount, much stronger influences are likely to be needed.
Even the study authors describe the effects as “limited, albeit encouraging”, clearly tempering expectations.
Why the researchers themselves urge caution
The authors are strikingly restrained in their conclusion. In essence, they say the effects observed are statistically demonstrable but small. It is still unknown how much these laboratory findings translate into differences in age-related disease or lifespan.
The team calls for further studies before anyone can market daily multivitamins as a reliable anti-ageing strategy.
In a piece in Nature, lead investigator Howard Sesso stresses that diet needs to be viewed as a whole. One particularly interesting question, he notes, is how a multivitamin compares with a deliberately healthier diet-yet there is currently very little data on that.
There is also the wider issue that epigenetic clocks are not yet a fully established standard. Many researchers consider them useful, but the precise relationships with chronic illness, diet, and supplementation remain incomplete. These lab-based clocks can indicate trends, but they do not replace long-term outcome studies on heart attacks, dementia, or cancer.
Who pays influences the picture too - the role of sponsors
Another factor that should not be overlooked when weighing the study is funding. Alongside public bodies such as the National Institutes of Health, several industry-linked organisations are listed:

- Pfizer Consumer Healthcare, now operating under the name Haleon

The authors state they had no conflicts of interest and remained independent. Even with such declarations, heavy industry involvement can still leave an uneasy feeling-especially as some organisations have previously faced criticism over questionable attempts to shape research or messaging.
If the same results had come from a purely publicly funded study, many experts would likely view them with markedly less scepticism.
Multivitamin or healthy cooking - what matters more in everyday life?
A key practical question that the COSMOS study touches only lightly is this: how does a pill compare with a consistently healthy lifestyle?
A realistic everyday scenario for older adults might look like this:

Who a multivitamin may be useful for
So does that mean: avoid multivitamins? It is not that simple. In certain circumstances, they can be sensible-particularly in later life-such as when:
- appetite and overall food intake decline
- illness or medication leads to nutrient losses
- dentures, chewing difficulties, or gastrointestinal conditions make absorption harder
- a one-sided diet cannot be avoided for a prolonged period
In older age, B vitamins, vitamin D, or particular trace elements such as zinc and selenium can become marginal. A well-chosen product can help fill gaps-ideally with medical guidance and occasional checks of blood results.
As an anti-ageing secret weapon, though, a multivitamin tablet is unlikely to qualify on current evidence. It looks more like a moderate safety net than a dramatic fountain of youth.

If you take a daily multivitamin, COSMOS suggests you might gain a small advantage on certain laboratory ageing clocks-around four months over two years. Whether that translates into more healthy, active years or a lower risk of heart attack, stroke, or dementia remains unresolved.
